If you have Adobe Acrobat and can fill out the forms right on the PDF, please do so and Email the completed PDFs and photos to Mary Lou Anderson, marylouanythingglassart@yahoo.com
with GUEST ARTIST APPLICATION written in the Subject Line.  

If you cannot fill them out right from the PDF, print the PDFs and fill them in by hand, and  please email Mary Lou Anderson, 
marylouanythingglassart@yahoo.com,
along with photos of your artwork with its medium, size and price, and inform her that you are interested in becoming a Guest Artist. You can bring the paperwork in when you visit the gallery to process your paperwork.
